If you live within our practice area (our practice boundary in 10 miles) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. The required forms are shown below.
All new patients are offered a health check (Over 16’s only unless you have existing significant medical condition) with a Practice Nurse or one of our Clinician Assistants. You do not need to see a GP for this appointment. This check will ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration.

When presenting to register you will need the following:
- HS200 for NORTHERN IRELAND & UK Residents ONLY. This form must be completed if you wish to register with our practice. You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. (Please refer to our registration policy below).
- HSC/R1 – This form is used for individuals entering Northern Ireland coming from any any other country OUTSIDE the UK. You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. Any person registering from outside the EU must submit a copy of their Visa or Work Permit.

If your application is refused
If you live over a certain distance away from a practice, or if it has closed its patient list, the practice may refuse your application. If this happens, you should contact the Business Services Organisation and ask them to put your name on a doctor’s list in your area.
Registering your baby
You need to fill in a GRO 4 Birth Registration Form. The General Registry Office will give you this when you register your baby’s birth. Bring this form to the GP’s surgery to register your baby as a patient.
Register online
Our online registration form will collect your details, and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
This online form can only be used if you wish to transfer to a GP within Northern Ireland or transferring from Great Britain. If this is your first ever registration in Northern Ireland, please download and complete the Application – first time registration form from below.
Please note, completion of the online form does not automatically register you with the Practice. Please wait on one of our team to contact you within in the next few working days to verify your registration, and answer any queries if required.

Drug Reduction Programme
The practice is participating in a Drug Reduction Programme along with other practices in the area. If you are currently on any of the listed medications this will be reduced and then stopped.

Organ Donation
Most people would accept an organ if they needed one. Not surprising really. Yet only 49% of us have signed the NHS Organ Donor Register.
Last year 11 people died in Northern Ireland waiting for an organ transplant. By signing the register and talking to your family about your decision you could help save the lives of up to nine people.
Please be advised when signing documents to become an Organ Donor patients must be 18 or over. Parents or guardians signing on behalf of under 18’s is not acceptable.
Changing Address
If you change your address please complete our online form with your new details. It if your responsibility to notify any Hospital you are attending or waiting to attend.
Please note – If you move outside our 10 mile Practice Boundary, you will have to register with a new GP Practice.
